Xray of Nonunion of Fracture of Scaphoid

The xray being shown belongs to 18 years old male who came to us after three months of initial injury. He complained of pain in his left wrist especially on lifting weight.

The patient had seeked medical help at time of initial injury but passed as soft tisue injury only.

The xray when he came to us shows an ununited fracture of the scaphoid bone.

The patient was treated successfully with open reduction, internal fixation and bone graft.

Note

It is a common occurence to miss the scaphoid fractures. The fracture line may not be visible at time of initial xary. A repeat xray at the interval of two weeks must be done if there is a tenderness in the scphoid area [Snuff box].

Role Of Cold Therapy in Pain Management

Cold therapy has been used for relief of pain since ancient times. In recent years, it is being used in the form of ice packs, vapocoolant sprays and ice massage.

It is most useful in acute musculoskeletal pain associated with sports injures or trauma.

Whenever cold stimulus (ice pack) is applied to skin, it melts and removes heat from tissues. This leads to vasoconstriction (The blood vesseles get constricted, their lumen and their capacity to carry blood increases),  reduction of nerve conductivity (As a result the nerve becomes less sensitive), reduction of muscle spasm and spasticity. Photograph of Traumatic Amputation of Hand

54 years old man presented to emergency with history of crush injury of hand in fodder cutting machine. All the fingers excluding thumb were amputated just few centimeters from base of the fingers [ All the amputations were proximal to proximal interphalangeal joint].

Multiple lacertated wounds over the dorsum of the hand also indicate towards hand being crushed.

Note

In rural areas it is very common to use fodder cutting machines which are driven by motor but need ot be fed manually. A dangerous combination because once the hand gets caught in the machine everything happens very fast. The arm gets sucked in and before person could call for help the trauma would have already occurred.

I have seen people with amputated forearms arms because nobody was around to stop the motor when their limbs were being chopped.

A purely manual machine in contrast is much safer. Because you need person to run it and it is slow as compared to motor driven, the danger is much less.

The shaft of the humerus lies between the upper border of the pectoralis major insertion proximally and the supracondylar ridge distally. This constitutes the middle three fifths of the entire humerus. The anterior portion of the greater tuberosity extends into an anterior ridge that ends at the coronoid fossa distally. The posterior aspect of the greater tuberosity continues distally as a lateral ridge that ends in the lateral supracondylar ridge. Xray of Non Union of Shaft of Fibula

The xray in the image is of 47 year old man who sustained fracture of the left fibular shaft after a minor fall. The xray is showing anteroposterior view of both the sides and on left side the fibular fractures ends have not united, have become narrowed and show a gap.

The xray is after 5 years of injury and patient does not have any complaint on the affected side. he had come to OPD for knee pain in the opposite knee and was investigated because he gave a history of trauma.

The xray in the image belongs to 23 year old man who sustained a fall fromm motorbike . The xray is laeral view of the knee and shows comminuted fracture of the patella.

The patient was operated ith tension band wiring and has recovered uneventfully.

Clinical Photograph of Blister Following Plaster Cast

The picture in example is of leg of 18 years old boy who had been treated for fracture tibia with plaster cast application. When plaster was opened after two weeks for relentless pain in the limb.

The patient’s limb was kept on Bohler Braun splint and put on calcaneal pin traction.

The fracture healed uneventfully.

Note: Plaster sore is a common complication of the plaster cast application. To minimize the complication, the plaster should be well applied and patient should be instructed to report immediately for any severe pain.
