What Is Iselin Disease or Traction Epiphysitis of Fifth Metatarsal Base

Iselin disease is the osteochondrosis or epiphyisitis of base of fifth metatarsal. It is also known as traction aophysitis or epiphysitis of fifth metatarsal base.

It was described by Iselin in 1912 as occurring in young adolescents at the time of appearance of the proximal epiphysis [growth plate] of the fifth metatarsal.

The affected epiphysis is a small, shell-shaped fleck of bone is located on the lateral plantar aspect of the tuberosity of fifth metarsal. It is not visible on anteroposterior or lateral radiographs and can be seen in the oblique view.

Peroneus brevis muscle insertion is in vicinity of this epiphysis.

The epiphysis appears in girls at about age 10 years and in boys at about age 12 years and is fused about 2 years later. [Read more...]

Bilateral Neglected Congenital Talipes Equino Varus – A Clinical Photograph

Talipes Equino Varus is a disorder of the foot where the ankle is in equinus (Bent so that toes move away from leg and heel comes closer.) and The heel is turned inwards so that sole faces inwards (varus).

In addition there is adduction of the front of the foot ( A movement that attmpts to bring great toe near the inner side of the ankle.

If no other cause is found, this condition is labeled as  Congenital Talipes Equino varus or CTEV.

It is one of the diseases that children are screened for at birth. It can be managed by manipulations and orthosis if detected early. Some cases require surgery.

It was very common ealirer to find people with neglected CTEV but the incidence has reduced.

This patient is 25 years old male  and has bilateral CTEV not treated out of ignorance.

The man had visited the OPD for some other ailments and did not want any treatment for this condition.

What Is Bunion?

hallux-valgusBunion is an enlargement of bone or tissue around the joint at the base of the big toe (metatarsophalangeal joint).th bunion is usually associated with hallux valgus [hallux refers to the great toe, "valgus" refers to the abnormal angulation of the great toe], a deformity of the toe where big toe may turn in toward the second toe. This leads to swelling and tenderness of the tissues and forms bump.

The bump can be swollen bursal sac and/or bony deformity.

For maintence of normal alignment muculotendinous forces  act. When there is imbalance tendons, ligaments, and supportive structures of the first metatarsal do not functioning correctly and lead to deformity.Hallux valgus, the primary deformity responsible for bunion can occur in any person and the cause is not known. Footwear has been implicated as a cause but needs to be substantiated.
