The xray below shows an isolated fracture of posterior malleolus of tibia.
Isolated Posterior Malleolus TIbia Fracture
Postrior malleolus fractures in isolation are less common than the fracture along with medial or lateral malleolus. Some times all of these are fractured, a fracture known as Trimalleolar fracture.

42 years old male with injury in motor vehicle accident.
Had comminuted fracture of distal tibia along with lateral malleolus fracture.
An open reduction and internal fixation was carried for both the fractures.
Tibia was fixed with distal tibia anatomical plate and fibula was fixed with one third tubular plate.

Bimalleolar Fracture With Ankle Subluxation With Tibifibular Diastasis
Note the separation of tibia and fibula [diastasis] represented by increased space between the two. The fracture of the fibula is quite high.
There is a subluxation of the ankle as well.

33 years old young swelling on the leg male got injured in a motor vehicle accident and presented to emergency with and inability to bear weight on the leg.
On assessment there were no injuries to any other part and his neurovascular status of the injured limb was normal.
The xrays of ankle revealed a trimalleolar fracture and a fracture line that extended up to tibial shaft.

These are the xrays of the patient with exposed plate in leg.
Here are the xrays after the injury. These xrays show comminuted fracture in distal tibia with fracture lines extending till articular area, something like Pilon fracture. Also seen is fracture of the fibula.
