32 year old man fell from motor cycle and broke his femur. Here is the anteropsterior xray.

Also visible in the above xray is malunited fracture of the upper part of the shaft of femur which on retrospective questioning was about 8 years old and treated conservatively.

Concomitant fractures of tibia and femur are increasing due to increase in motor vehicular trauma. The following images are radiographs of a young man who suffered from injury to his femur and tibia when his bike struck with an auto-rickshaw and he was toppled over.
Following xray is of the thigh bone. A fracture in middle third – lower third is visible along with a butterfly fragment.

Oblique fractures result due to indirect trauma on the bone.Femoral fractures require a major trauma to occur.
The following xray belongs to a 13 year old child who fell from height. Obliquity of the fracture is very clearly visible in this view. The bone has broken at upper third middle third junction.

The Fracture Shows A Good Union
Distal femur fractures are also called intercondylar fractures and supracondylar fractures. Former are intra-articular injuries and often need surgical stabilization.
This is an xray of patient who was treated surgically for intercondylar fractures with use of condylar plate as implant.
The fracture has united very well and articular congruity is very well maintained.

Intercondylar fractures of femur occur after substantial trauma. The xray is of 37 years old male who sustained this fracture following road traffic accident.

Intercondylar fracture of femur causes severe knee swelling as it is an intra articular fracture.
Also visible in the image is Cramer splint.

Broken inrameduallry interlock nail in femur fracture. Implant has failed before union of fracture
Intramedullary interlocked nail is a very common implant used for treatment of diaphyseal fractures of long bones. It allows early mobilization of the limb owing to stable fixation. The implant is called interocked because screws are used on eitherside to lock the implant to the bone.
Implant failure is one of the problems associated with implant surgery. In this the impant metal gets fatigued before a fracture could have united. It is basically a race against time between fatigue of the implant and union of the fracture.
Two of the major causes of implant failure are poor implant [In terms of design, strength etc.] and poor surgical technique.
In the present image the implant has broken at the fracture site and the fracture has not united. The patient was 48 years old and the implant broke after 4 months of surgery. [Read more...]

DHS and Interfragmentary screw in subtrochanteric fracture
Subtrochanteric factures are fractures below the intertrochanteric line of femur. These are high velocity fractures and quite common in young active adults.
The xray in the picture is of a 43 years old man who sustained this fracture after fall from height.
The xray is one month following the surgery and fracture is showing union in most of its surface.
Fractures of subtrochanteric region are not amenable to surgical treatment and are treated by operative methods.

Fracture of Shaft of Femur
The picture shows fracture of shaft of femur of right side at upper and middle third junction. Fracture femora are high energy injuries and are quite common in young, active individuals.
Also visible in the xray is poorly applied Cramer Wire Splint. Because it does not span one joint above and one below the fracture, it is a poor splint.
