Following xray is of 28 years old male who was operated with dynamic compression plating.

The xray was taken after 4 weeks of surgery.
The man with following xray cam to OPD for removal of plate in the upper ulna as it caused discomfort to him. Other injuries in the same forearm were radial head which was fixed by miniplate, radial shaft fixed with locking plate and distal ulnar plate.

Radial shaft fracture had not united at the moment [Seven months after the surgery. He was suggested bone grafting procedure for that fracture but he decided to go ahead with just imlpant removal.

Fracture of Radius Bone
Such a fracture in adult always need surgical fixation.
Following is a phtograph of froearm of 43 years old male who was operated for fracture in ulnar bone and in postoperative period got infected.
The infection was controlled with help of debridement and antibiotics as per his available records and the plate got exposed.

Exposed Plate In Ulna
The photograph reveals quite well the exposed ulna and the plate. An area in the middle shows the fracture line and the surrounding bone is devoid of any tissue cover.
The patient has presented to the OPD only one time and failed to follow up.
Note: This kind of problem is quite complex and because the fracture has not united, the situation is one of the difficult situations faced in the orthopedics.
The treatment requires control of any lingering infection, thorough debridement, a flap surgery to provide cover to the bone and if required removal of present implant & temporary fixation which is to be followed by definite fixation and bone grafting at a second stage.
Treatment as always needs to be individualized.
Comminuted fractures are generally as a result of direct trauma to the bone. Following xray is anteroposterior view of the forearm and is revealing fracture ulna. A comminuted fragment is seen in the ulnar fracture.

Monteggia fracture involves fracture of shaft of ulna and dislocation of radius. Following xray is of 39 years old male who suffered a motor vehicle injury to his upper limb and found to have a Monteggia fracture.

Type I Monteggia Fracture Dislocation
AS per Monteggia fracture classification, the fracture is Bado type I.
The fracture was treated by open reduction and internal fixation using low contact dynamic plate and transfixing the radial head to ulna.
