47 years old mason fell on a pavement and his forearm struck the pavement. He presented with swelling in the forearm and severe pain Xray of the forearm revealed fracture of ulna.

He presented with pain in right arm and swelling in upper third of arm.
Xray revealed fracture of upper third of radius with a small comminuted fragment (butterfly) [As depicted by arrow]

The patient was offered open reduction and internal fixation with low contact dynamic compression plate but the patient refused for any treatment.

This photograph is of forearm of 54 years old patient who was admitted to the hospital for some medical problem.

The deformity in the left forearm was quite prominent.

She gave a history of trauma19 years ago and there was fracture of both the bones [radius and ulna]. The fracture was managed by an osteopath because she refused to take operative treatment.

Interstingly, the fractured fragments were mobile and I wanted take an xray picture to look for pseudoarthrosis but she refused.

Also, in the picture, you can see patchy skin depigmentation.

22 years old female had an accident while she was driving a scooter that resulted in injury to her right [dominant limb] forearm. Examination also revealed posterior interosseus nerve palsy.

Xray revealed fracture of proximal third of radius with mid shaft ulna.

Fractures of forearm bones are very common in paediatric and young age group. While in children they are amenable to closed reduction and plaster application, in adults they almost always require surgical treatment.

The image belongs to a 40 years old man who sustained trauma to left forearm when his motor bike collided with another vehicle. The man also had fracture of pubic ramus along with this injury.

The patient was operated for his forearm injury and fracture pubic ramus was manged conservatively.

Posttraumatic Radioulnar Synostosis Follwing Forearm Fractures

Synostosis of the radius and ulna following fracture is relatively uncommon.The incidence is between 2-6% of the fractures. Very badly comminuted fractures occurring at the same level are at higher risk of developing this complication.me level. A greater association of this complication has been noted in patients who had crushed injury of forearm or there was an associated head injury.

Proximal fractures of forearm bones treated with one incision are more prone.

Because synostosis causes a bony bridge between radius and ulna it causes loss of rotation movements of forearm.

Operative Treatment of Monteggia Fracture Dislocation

Adult Monteggia fractures rarely yield to closed reduction. Most of the time operative treatment is needed. The goal is to fix ulna in anatomical position and reduce radial head. Ulna is usually fixed by a 3.5-mm limited-contact dynamic compression plate .

Monteggia fractures should be operated as soon as feasible. patient is placed in supine or lateral position depending upon surgeon’s convenience.

After stabilization of the ulna, the elbow is passively ranged to assess the stability of the radial head. An image intensifier can help assess radial head stability. After the surgery wound is closed in layers without closure of fascia.

If there is a radial or posterior interosseous nerve palsy that has occurred with injury, the course of action depends on reduction of radial head. if the reduction of radial head occurs easily, exploration of the radial or posterior interosseous is not indicated. [Read more...]
