Avascular Necrosis With Head Collapse in Operated United Fracture Neck of Femur

This is xray of an operated fracture neck of femur that went into avascular necrosis in spite of union. This 37 years old male was operated about 7 months prior to the time this xray was taken. His chief complaint was pain and stiffness  in the affected hip.

The xray  shows that the fracture of neck of femur has united but there is deformation of the head with fragmentation and collapse . There is a sclerosis of the head too. Overall findings suggest avascular necrosis of the head of the femur.

Implant Failure – Xray of Broken Assembly of Dynamic Hip Screw In Intertrochanteric Fracture

82 years old male who had undergone surgery for intertrochanteric fracture of femur on his right side about 9 months back came to out patient department with complaint of knee pain.

He used to walk with help of a walker.

On examination his knee was found to be normal and xrays of both hip and knee were ordered. While his knee xrays were unremarkable, xray of his hip revealed a broken dynamic hips screw and side plate assembly.

All the screws of the sideplate are broken and fracture is not united too.

Note: Failed DHS like this one is a difficult sitauation in elderly people. While refixation is an option, another option is hip replacement.

Fracture Bilateral Neck of Femur In A Woman With Post Polio residual Paralysis

25 years old female presented to the OPD with complaint of pain in left hip region. She walked with a bilateral lurching gait.

Examination revealed bilateral lower limbs which appeared hypotrophied with right more than left. Hip movements were exaggerated in all the planes and both hips were found to be unstable.

The xray revealed following picture.

The xray shows  old fracture of neck of femur on both the sides. Bilateral acetabuli are malformed and right femur is more hypoplastic than left.

There was no history of any trauma.

A diagnosis of long standing bilateral neck femur fracture with probable cause as childhood osteomalacia was made.

Patient was managed on analgesics and local measures like hot fomentation.
