A 33 years old male suffered trauma when he fell from motorbike. He had a closed fracture of lower third of tibia on right side. His xray after injury [Limb is splinted as you can see] showed a spiral fracture of tibia along with fracture of fibula of right side.
The patient was treated with open reduction and osteosynthesis using plate and screw. A screw was also used to hold the fracture before plate was applied. Fibula was not fixed as length was regained as checked under image.

The second xray is one month following the surgery. The fracture eventually united well and patient has regained his prefracture range of motion.
Spiral fractures need to be treated with good reduction and plate and screw with compression screws offer very good option for these fractures.
