Avascular necrosis (AVN) of head of femur occurs when blood supply to the head is jeopardized. That may occur when blood vessels are injured or the pressure within the bone rises enough to cause narrowing of blood vessels resulting in ischaemia of the bone.
The various known causes of avascular necrosis of hip are
- Trauma
- Prolonged steroid use
- Excessive alcohol use
- Gaucher’s disease
- Pancreatitis
- Radiation treatments and chemotherapy
There are percentage of people in whom the cause cannot be determined and are labeled as idiopathic AVN.
A 39 years of young mail, known alcoholic presented with pain in both the thighs for past 7 months. He had taken various kinds of non allopathic treatments for the problem but did not improve.
Movements of both the hips were restricted and painful.
An xray pelvis including both hips revealedĀ avascular necrosis of both the femoral heads with arthritic changes bilaterally.
The disease was quite advanced in both sides involving acetabulum as well. Note the sclerosis [The bone appears whiter than rest] in both the heads.
The left side also shows fragmentation of the head.
The patient was offered treatment by bilateral replacement of hip. The patient is still considering it and meanwhile is on analgesics and life style modification.
