The xray is done after 1.5 years of surgery and fracture is well united. Osteoporosis of the bones is quite visible.

Lag screw is the thick screw in the head. If you note carefully, it is protruding beyond the margins of the head.

The assembly was removed because patient had pain on weight bearing.

Probably, the fixed angle blade plate was done as a revision surgery because there are broken screws in addition to screws that are holding the plate.

There was no patient information attached with this file.

Note Fixed angle blade plate was extensively used earlier for treatment of intertrochanteric fractures but with better gadgets available now, it is rarely used.

Implant Failure – Xray of Broken Assembly of Dynamic Hip Screw In Intertrochanteric Fracture

82 years old male who had undergone surgery for intertrochanteric fracture of femur on his right side about 9 months back came to out patient department with complaint of knee pain.

He used to walk with help of a walker.

On examination his knee was found to be normal and xrays of both hip and knee were ordered. While his knee xrays were unremarkable, xray of his hip revealed a broken dynamic hips screw and side plate assembly.

All the screws of the sideplate are broken and fracture is not united too.

Note: Failed DHS like this one is a difficult sitauation in elderly people. While refixation is an option, another option is hip replacement.
