Implant Failure – Xray of Broken Assembly of Dynamic Hip Screw In Intertrochanteric Fracture

82 years old male who had undergone surgery for intertrochanteric fracture of femur on his right side about 9 months back came to out patient department with complaint of knee pain.

He used to walk with help of a walker.

On examination his knee was found to be normal and xrays of both hip and knee were ordered. While his knee xrays were unremarkable, xray of his hip revealed a broken dynamic hips screw and side plate assembly.

All the screws of the sideplate are broken and fracture is not united too.

Note: Failed DHS like this one is a difficult sitauation in elderly people. While refixation is an option, another option is hip replacement.

Anteroposterior View of Intertrochanteric Fracture of Femur

Intertrochanteric fractures of femur are typically osteoportic fractures and are more common in elderly people. they are more commonly reslut of of insufficient trauma. Usual history is that of fall from standing height. Slippery floors or bathrooms are added factors.

The xray shows one month old fracture in 78 years old lady. The lady presented to us one month after the injury. An attempt at repair is visible in form of callus.

The aim of treatment in these elderly patients is to stablise the fracture surgically and make the patient upright so that complications of recumbency are avoided.

this patient was successfully operated with dynamic hip screw.

Xray of Non Union of Shaft of Fibula

The xray in the image is of 47 year old man who sustained fracture of the left fibular shaft after a minor fall. The xray is showing anteroposterior view of both the sides and on left side the fibular fractures ends have not united, have become narrowed and show a gap.

The xray is after 5 years of injury and patient does not have any complaint on the affected side. he had come to OPD for knee pain in the opposite knee and was investigated because he gave a history of trauma.

Fibrous Dysplasia of Humerus-An Xray

Following image shows an xray of right shoulder with humerus showing firbrous dysplasia. The xray is of a young girl, 23 years old who presented with pain in right arm for about one year.

The xray shows a large lytic lesion that is extending from just below shoulder joint to middle of the humerus. Multiple septae are visible and outer cortical rim of the bone is maintained, suggesting a benign pathology.
